Sacramental Certificate Request Form

A $10 fee is charged for certificates issued from the Archdiocesan Archives and Records Office. (A fee waiver may be made in the case of financial hardship.)

Certificates of records after 1930 are available only to the person named in the record (or legal guardian if under 21) or to church personnel for sacramental purposes. Others need a signed release from the person named in the record. Certificates for sacramental purposes are generally filled within 2 business days; other requests are filled as resources permit.
