Sacramental registers are retained at the parish. Individuals in search of one’s own sacramental record from an active parish must contact that parish directly to obtain their record. Certificates can ONLY be issued for sacramental or legal purposes.
Certificates of records can only be issued to (a) the individual requesting his/her record, (b) the legal guardian if under 21 years old, or (c) church personnel for sacramental purposes. If you are requesting someone else's record, please call us at 314-792-7020.
Requests made by government or corporate agencies (e.g. Social Security Administration, immigration, insurance companies etc.) must be accompanied by an Authorization For Release of Information signed by the person (or person's legally qualified guardian) whose record is being requested. If you are an individual requesting your own certificate, please continue to read the directions and fill out the form at the end of this page.
Obtaining Certificates from Closed Parishes
The Archdiocese of Saint Louis Office of Archives and Records assumes custody of sacramental records once a parish is closed. These records are centrally located in the Office of Archives and Records. There are a few exceptions where the registers of a closed or merged parish are retained at another parish.

Payment and Identification
Sacramental Certificates are $10 each and will be mailed within 5-10 business days. Certificates will be sent via regular mail unless otherwise requested.
Payment can be made via cash, credit card or check, made out to the Archdiocese of St. Louis Archives. At the end of this form, you may pay online with your credit card by following the links. YOU MUST SUBMIT A COPY OF YOUR DRIVER'S LICENSE along with this request by email, mail, fax or by attaching it to the bottom of this form before selecting "Submit." We will not release records without proof of identification.
