Archdiocesan Employee Free Vaccine Program

Flu vaccines were provided FREE to Archdiocesan employees, spouse, and their dependent children.  

If you missed your opportunity to receive the free flu vaccine, please contact your provider for an appointment. You can also call UnitedHealthcare customer service 1-888-332-8885 to request providing retail locations such as Target or Walgreens that particpate with your UnitedHealthcare plan.

Using Employee Self-Service Sign On

Here are a few steps to follow in order to make the Employee Self Service process easier:

   1. Have your User Name and Password available. To create a new password, click on the green button below.

   2. Internet Explorer 6.0 or higher and a Windows PC computer is required. No Foxfire, Mozilla or Macs.

   3. Now you are ready to access the Employee Self-Service:

You can click on (new window) Self Service Sign On.

Please note: Popup Blockers must be disabled in order to use the Self Service Sign On. On your toolbar of your Internet browser, click on "Tools", a drop down list should state "Pop Up Blockers". Follow the process to turn off the pop up blocker.
