This form may be used to submit research requests or general questions to the Office of Archives and Records.
If you need a baptismal or sacramental certificate, please use the Sacramental Certificate Request form.
For transcripts, please use the Transcript Request form.
For research requests, please fill out the form below. A staff member will respond to you within 7 business days. Generally, we will provide reference services for free up to an hour. If the research is very involved, we may ask you to visit the archives in person to conduct your research, or charge research fees at $25 per hour. We will not proceed with paid research until discussing it with the researcher. Please do not send payment without discussing it with staff. While we do not have set fees, we encourage you to donate for reference services.
