This form should be used only to make general or research requests.
If you need a baptismal or sacramental certificate, please use the Sacramental Certificate Request form.
For transcripts, please use the Transcript Request form.
Due to high request volume, staff will provide reference services for free for up to an hour. Additional hours required will be charged to the requestor at $25 per hour. Staff will not proceed with paid research until discussing it with the researcher. Please do not send payment for research hours until you've discussed it with a staff member.
