Voluntary Life Insurance Plan with Hartford Life

Employee: You can enroll for life insurance coverage for yourself in increments of $10,000 up to $300,000 (note the Guaranteed Issue Amount below for new hires only). If you elect the insurance coverage, you may also elect coverage for your spouse and/or your child(ren) in the following amounts:

Spouse: Increments of $5,000 to a maximum of $150,000, subject to a maximum of 50% of the employee's approved amount of coverage.

Child(ren): Increments of $5,000 to a maximum of $15,000.

Guaranteed Issue Amount

If you enroll for coverage within 31 days of your date of hire, you are guaranteed coverage up to the amounts shown below. You will be required to submit a Personal Health Application for Hartford Vol Life Insurance  if you enroll for coverage above the guaranteed issue amount. Hartford Life will communicate directly to you regarding the Personal Health Statement and approval.

Employee: $100,000                 Spouse: $25,000                      Child: $15,000

After your first 31 days as a new hire and if you do not have life insurance coverage or you want to increase your current Hartford Life Insurance, you can apply anytime throughout the year. You could enroll or change voluntary life insurance coverage for yourself via Hartford's enrollment forms, the Hartford Life Enrollment_Change_Cancel Form 2013 - 2016 and the Personal Health Application for Hartford Vol Life Insurance . Give the completed forms to your Benefits Administrator to process. Hartford would then review your forms for approval and notify you. Hartford will have a rate increase effective July 1, 2013. After July 1, 2013 please use the Hartford Life Enrollment_Change_Cancel Form 2013 - 2016 for any enrollments/changes/cancellations after July 1, 2013. 

Beneficiary Designation

After enrolling online for the life insurance plan, please make sure that you go back to the main screen for Open Enrollment and assign beneficiary(ies). For spousal or child life coverage, benefits payable for a spouse or child(s) death are payable to you, if living. If you are deceased, Hartford may pay the benefit to your surviving spouse or to the executors or administrators of the estate.  A beneficiary for employee Life Insurance may be changed upon written request.
