Position Description: 

Functions as Minimum Data Set (MDS) Care Plan Coordinator for Mary, Queen and Mother Center. Ensures the MDS for all new admissions, re-admissions, annuals, quarterlies, and change of condition are completed by the interdisciplinary team within Omnibus Budget Reconciliation Act (OBRA) guidelines.

1. Coordinates the completion of all MDS forms with the multidisciplinary team.
2. Responsible for inputting MDS and care plan information into the computer and updating as needed.
3. Ensures completion of MDS forms for all new admissions, re-admissions, and change of condition
for all residents, within OBRA guidelines, including Prospective Payment System (PPS) and MDS.
4. Coordinates the completion of all annual and quarterly MDS forms.
5. Responsible for transmission of MDS information.
6. Coordinates the pre-MDS meeting and care plan schedule with interdisciplinary team.
7. Attends care plan conferences to present and update care plan and MDS information including
unscheduled family care conferences.
8. Generates/updates any needed reports or files including Health Care Finance Administration
(HCFA) census/resident roster reports as needed.
9. Works with Charge Nurses to maintain a working knowledge/familiarity of all residents.

11.Screens potential admissions and coordinates with Therapy Department on all Medicare
12.Coordinates with Therapy Department the dates for completion of PPS and keeps billing informed
of current Resource Utilization Group (RUG) rates.
13.Responsible for ensuring staff follows HR Policies and Procedures.
14.Performs other duties as assigned.

1. Must be a Registered Nurse with a current Missouri license.
2. Must have a working knowledge of MDS requirements according to OBRA guidelines and care plan
requirements, including the PPS for Medicare.
3. Must have experience in geriatric Nursing care and care of the chronically ill and convalescent
4. Must have knowledge of State and Federal regulations governing extended care facilities.

1. Typing, data processing, and computer skills.
2. Good nursing assessment skills.
3. Specialized interest in geriatric Nursing.
4. Must be a self-starter, able to plan and organize own work on a daily basis.
5. Must be tactful, courteous, and possess good communication skills.
6. Must have good physical and emotional health.
7. Must be willing to promote the philosophy of MQMC.
8. Assists with staffing and scheduling as needed.
9. Must be flexible with scheduling to ensure proper staffing is met on all shifts.
10.Functions in any Nursing capacity to meet the needs of the residents.
11.Must be able to prioritize work.
12.Must be willing to take wee-end call and work as Nursing Administrator and/or MQMC Week-end

Requires contact with the Medical Records Department, Nursing staff, and members of the interdisciplinary team responsible for the completion of the MDS, Medical Director, staff physicians, staff, visitors, families, and other medical consultants.

Responsible for ensuring that appropriate care is provided by Charge Nurses, CMTs, and CNAs. Ensures all Nursing policies and procedures are followed.
